2024 Sewall Diagnostic Clinic Application
From June 2021 to July 2022, Sewall’s Diagnostic & Evaluation Clinic conducted approximately 66 diagnostic evaluations for children from 11 months through 16 years of age. Sewall is proudly one of the only clinics providing multidisciplinary, arena-style evaluations in the larger metro area, and one of two serving Medicaid-eligible families. Last year, 57 of the 66 children evaluated were Medicaid-eligible.
By following all CDC guidelines, the vast majority of evaluations were able to be performed in person during the COVID-19 pandemic. Sewall is proudly one of the only clinics providing multidisciplinary, arena-style evaluations in the larger metro area, and one of two serving Medicaid-eligible families. Last year, 43 of the 47 children evaluated were Medicaid-eligible.
Evaluation Information
- Sewall’s Diagnostic & Evaluation Clinic (D&E Clinic) performs evaluations at Sewall’s main location, 940 Fillmore Street in Denver.
- We provide developmental evaluations for children from age 18 months through early teenage years.
- We specialize in testing for possible autism and overall developmental delays, which include speech-language, cognitive, sensory, and gross- and fine-motor delays.
- Our team provides a multidisciplinary, group-style evaluation that combines the expertise of a clinical psychologist, speech-language pathologist, and occupational therapist.
- We accept Open Colorado State Medicaid and CHP+ Medicaid which cover the cost of the evaluation.
- Currently, we do not have contracts with Denver Health Managed Care Medicaid or private insurance companies.
- We do accept private pay; the cost of a Sewall evaluation is $3,500.
- Evaluations are performed every Tuesday; the time your child will spend on the wait list can vary from a few weeks to a few months.
Records Needed for Evaluation
- Medical records from your child’s Primary Care Provider (PCP)
- Birth records from your child’s birth hospital
- Most recent IFSP or IEP and evaluation report
- Speech, motor, cognitive, and mental health (if applicable) evaluations or therapy notes
- Family Services Plan, Part 2 Family History, if the child has been placed in child protective services
It is helpful if you are able to secure any of the above records yourself and include them with your child’s application; otherwise, we will send for records using your signed Release of Information that’s included in the application.
